The HSE in court over its chemical classification fast-track
An environmental group has filed for judicial review of the 2026 regulations that gave GB a fast-track route for adopting other territories' chemical hazard classifications. The dispute is about who decides what counts as a hazard — and on whose evidence — which is upstream of the data every study team relies on.

When the safeguards are recommendations: the CSB on Yenkin-Majestic
Three years after a fatal 2021 resin-plant explosion, the CSB says Yenkin-Majestic/OPC Polymers has not implemented or substantively responded to any of its five safety recommendations, despite two follow-up letters and a federal guilty plea. The underlying findings — a reactor manway installed without design or testing, no evacuation alarms — map straight onto mechanical integrity and emergency-response layers.

A reacting monomer in an intermodal tank: the Toledo glycidyl methacrylate release
A roughly 5,500-gallon tank of glycidyl methacrylate at a Toledo transfer terminal began releasing on the night of 20 October 2026, forcing a one-mile evacuation and a three-mile shelter-in-place. Ohio EPA's early read points to an error during material transfer; responders ultimately let the reacting chemical finish rather than approach the tank. For study teams it reads as a reactive-chemistry and transfer-operations case.
